Commercial Driver's License (CDL) Hazardous Materials Endorsement
License Description Minnesota Department of Public Safety (DPS) In most cases, must be 21 years old to haul hazardous materials intrastate. Be 21 years of age for interstate transportation. Be physically qualified. Complete Commercial Driver's License self-certification form and provide a valid U.S. DOT Medical Examiners Certificate if applicable. _________ Complete a Security Threat Assessment, including collection of fingerprints at time of adding HazMat endorsement, transferring from another state, upgrading to a higher class CDL, and at time of renewal.        Security Threat Assessment Application:          -Transportation Security Administration (TSA) Application Fee: $86.50  Refer to TSA website for more information on the federal regulations.           -You must complete a Security Threat Assessment Application before submitting your fingerprints at an authorized collection site.       Online - visit Hazardous Materials Endorsement     By Phone - Toll free 855-347-8371   The TSA is responsible for the screening and the approval or rejection of each background check. You will be notified by TSA if you are ineligible for a HM endorsement. For more information regarding disqualifying offenses and the appeal process, visit the TSA.  _________ Pass CDL knowledge and endorsements tests. Pass a road test in a commercial vehicle of a size applicant intends to drive.  Pay fees and make application. Have no disqualifying convictions on driving record. One license issued, others must be invalidated.
