X-ray Supervisor and Operator Certificate/Permit
License Description Supervisor and Operator Certificates and Permits are issued to licentiates of the Healing Arts in the following fields: Medical physicians and surgeons, osteopathic physicians and surgeons, podiatrists and chiropractors. CERTIFICATES AND PERMITS Radiology Supervisor and Operator Certificate Fluoroscopy Supervisor and Operator Permit Radiography Supervisor and Operator Permit Dermatology Permit FEES Application Fee: $ 127.00 per category Renewal Fee: $ 104.00 per category (two-year renewal period) APPLICATION REQUIREMENTS Radiology Supervisor and Operator Certificate: (1) Submission of required application form, non-refundable application fee and (2) Provide evidence of certification by the American Board of Radiology or the American Osteopathic Board of Radiology. Examination is not required for Radiology Supervisor and Operator applicants that meet the above criteria. Supervisor and Operator Permits: (1) Submission of required application form, non-refundable application fee and (2) Provide evidence of one of the following valid California healing arts licenses: (a) Physician and surgeon, (b) Osteopathic physician and surgeon, (c) Podiatrist or (d) Chiropractor. Candidates must pass CDPH-RHB approved examinations in the category for which the application was submitted as well as radiation protection and safety. EXAMINATIONS Examinations are administered by ARRT on behalf of CDPH-RHB. ARRT determines examination fees. PearsonVue Testing Centers are located throughout the United States.
