Certified Phlebotomy Technician II (CPT II) Certificate
License Description Fees: Fees are subject to change with each fiscal year. Refer to the LFS General Fee Information Page for a full list of fees associated with licensing. Exams: Applicants must refer to the LFS Website for the list of approved certifying organization examinations for each license category. Academic, Training, and Experience Requirements: High school diploma from an accredited high school in the United States or equivalent plus a current, valid holder of a Certified Phlebotomy Technician I certification from the California Department of Public Health or meet the Certified Phlebotomy Technician I certification requirements; have at least 1040 hours of acceptable on-the-job phlebotomy experience in the past five years; and complete 20 arterial punctures that fulfill all sampling requirements of all clinical laboratory tests or examination.
